* To mark your chart for urine output:
Put one plus in the box on your chart for each wet diaper that has about 1/4 cup of liquid. Sometimes the diaper is clearly not wet enough to contain that much liquid. If that happens, put one line of the plus sign.
If another diaper in that 24 hour period also is a bit light put the second line of the plus.
If the wet diaper feels heavy enough to have 1/2 cup of liquid, put two plus signs.

**To mark your chart for bowel output:
Put a slash through an S when your baby's bowel movement contains some mass(more than a stain on the diaper).

Comments:

Three Skills for the learning Time:
1. Keep a record of your baby's urine output. Stop tracking when the baby has met the goal for the time period.
2. Keep a record of your baby's bowel movements.
3. Keep a record of the feel of your breasts before and after a nursing session.

Three signs from the baby that show help is needed:
1. Your baby is not having the minimum number of wet diapers for two days in a row, GET HELP.
2. Your newborn (under two weeks of age) is not having the amount of bowel movements you would expect for two days in a row. GET HELP.
3. Your baby is fretful at the breast and doesn't seem to enjoy breastfeeding. GET HELP.

Three signs from the mother that show help is needed:
1. You don't notice an increase in firmness of your breasts and your baby is missing the milestones for wet or soiled diapers. Seek advice right away.
2. Your breasts continue to get more and more engorged as the days go by, or soften only a little when your baby suckles. Get lactation assistance.
3. You have sore nipples. Look for help.
